Lost or Stolen Cardinal Card

Lost or stolen cards or unauthorized card use should be reported immediately as follows:

1. Online at Cardinal Card Account Manager
2. Contact the ID office Monday through Friday 8:00AM - 4:30PM at 518-564- 2035 or toll free 1-877-778-CAS1(2271).

REPLACEMENT FEES

1. The fee to replace a damaged card is $5.00 (you must present the damaged card to receive this discounted fee)
2. The fee to replace a lost or stolen card is $15.00
